Form 1099-DIV

This worksheet is used to enter the taxpayer's and spouse's income reported on a Form 1099-DIV, Dividends and Distributions. Information reported on Form 1099-DIV is automatically calculated and reported throughout the tax return on different forms and schedules.

 

Missing Form 1099-DIV
The Payer of the income is required to send a Form 1099-DIV to the taxpayer or spouse to report dividends and/or distributions. If the taxpayer or spouse has not received a Form 1099-DIV, and believes he or she should have, the Payer of the dividends should be contacted immediately.

 

To report more than one Form 1099-DIV
Enter the information for the first Form 1099-DIV and then click the "Save" button at the bottom of the worksheet. The user will be returned to the Worksheets screen. Click the Add button next to "Form 1099-DIV" to open a new Form 1099-DIV worksheet. Enter the information for the second Form 1099-DIV and repeat the process until all Form 1099-DIVs have been entered.  

 

 Note: Do not make an entry, such as "0" or "N/A" for boxes that are blank.

Payer and Recipient Information
Payer's Name Enter the Payer's Name exactly as it appears on the Form 1099-DIV received from the Payer.
Taxpayer or Spouse Select whether this Form 1099-DIV belongs to the taxpayer or spouse.
Payer's Federal ID Enter the Payer's Federal ID number exactly as it appears on the Form 1099-DIV received from the Payer.
Recipient's SSN and Name This information will be autopopulated when the user selects Taxpayer or Spouse above.

 

Enter the correct name as entered on the Form 1099-DIV received from the Recipient only if it does not match what has been autopopulated for the Recipient.

Payment and Tax Withholding Information
Box 1a Total Ordinary Dividends
Enter the amount of Total Ordinary Dividends exactly as reported on the Form 1099-DIV.

 

This box includes ordinary taxable dividends received from the Payer.

Box 1b Qualified Dividends
Enter the amount of Qualified Dividends exactly as reported on the Form 1099-DIV.

 

This box includes dividends from Box 1a above that may be eligible for the 15% or zero capital gains rates. The taxpayer's allowed rate will be automatically calculated.

Box 2a Total Capital Gains
Enter the amount of Total Capital Gains exactly as reported on the Form 1099-DIV.

 

This box includes total capital gain distributions from a regulated investment company or a real estate investment trust.

Box 2b Unrecaptured Section 1250
Enter the amount of Unrecaptured Section 1250 amount exactly as reported on the Form 1099-DIV.

 

This box includes capital gain distributions from Box 2a above that are unrecaptured section 1250 gain from certain depreciable real property.

Box 2c Section 1202 Gain
Enter the amount of Section 1202 Gain exactly as reported on the Form 1099-DIV.

 

This box includes Section 1202 gains from certain qualified small business stock which may be eligible for a 50% or 60% exclusion. The taxpayer's exclusion will be automatically calculated.

Box 2d Collectibles Gain
Enter the amount of Collectibles Gain exactly as reported on the Form 1099-DIV.

 

This box includes capital gain distributions from Box 2a above that are from the sale or exchange of collectibles, which are calculated at a 28% tax rate.

Box 3 Nontaxable Distributions
Enter the amount of Nontaxable Distributions exactly as reported on the Form 1099-DIV.

 

This box includes any part of a distribution reported above that is nontaxable because it was a return of the cost paid for the investment or other basis adjustment.

Box 4 Federal Tax Withheld
Enter the amount of Federal Tax Withheld exactly as reported on the Form 1099-DIV.

 

This box includes federal taxes withheld by the Payer and paid directly to the IRS by the Payer on the taxpayer's behalf.

Box 5 Investment Expense
Enter the amount of Investment Expense exactly as reported on the Form 1099-DIV.

 

This box includes the taxpayer's share of expenses of a nonpublicly offered regulated investment company that may be deductible on Schedule A subject to the 2% limit. The taxpayer's allowed deduction will be automatically calculated.

Box 6 Foreign Tax Paid
Enter the amount of Foreign Tax Paid exactly as reported on the Form 1099-DIV.

 

This box includes foreign taxes paid on the investment which may deductible. The taxpayer's allowed deduction will be automatically calculated.

Box 7 Foreign Country
Enter the Foreign Country to whom the tax was paid exactly as reported on the Form 1099-DIV.

 

This box shows the foreign country that collected the taxes paid by the investment company on the taxpayer's behalf.

Box 8 Cash Liquidation
Enter the amount of Cash Liquidation Distribution exactly as reported on the Form 1099-DIV.

 

This box shows the amount of a cash distribution received by the taxpayer as the result of a liquidation of the investment.

Box 9 Noncash Liquidation
Enter the amount of Noncash Liquidation Distribution exactly as reported on the Form 1099-DIV.

 

This box shows the value of a noncash (property) distribution received by the taxpayer as the result of a liquidation of the investment. The value is determined by the Fair Market Value of the property on the date of distribution.

Box 10 Exempt-Interest Dividends
Enter the amount of Exempt-Interest Dividends exactly as reported on Form 1099-DIV.

Check the box if it is checked on the Form 1099-DIV which indicates that this amount is taxable on the state return.

This box shows the value of exempt-interest dividends from a mutual fund or other regulated investment company paid to the taxpayer during the tax year.

Box 11 Specified Private Activity Bond Interest Dividends
Enter the amount of Specified Private Activity Bond Interest Dividends exactly as reported on the Form 1099-DIV.

 

This box shows the value of specified private activity bond interest subject to the alternative minimum tax.

Box 12 State
Select the state for which state income tax has been withheld from the dropdown list.
Box 13 State ID No.
Enter the State ID No. exactly as it is reported on the Form 1099-DIV.
Box 14 State Tax Withheld
Enter the amount of state tax withheld exactly as reported on the Form 1099-DIV.
